I saw lots of YouTubers playing L4D2 with third person on Versus mode. It should works with sv_cheats 1. Otherwise, even if thirdpersonshoulder is set to 1 you'll only have the first person view. Also, some servers will kick you out for having thirdpersonshoulder 1, so set it back to 0 in order to play on them.
